The Middle School sports program does require a Pre-Participation Physical Evaluation. Sports physicals are mandated by the NJ Department of Education for all student athletic sports, intramural sports, and marching band. They must be completed on the state form by your child's physician and by the parent. Our school physician, Dr. Andrew Nelson, reviews these forms before sports begin. He requires the form that the student's physician originally completes to be turned in (not a copy). Packet #1 instructions: Pages 1-2 of the Pre-Participation Physical examination are to be completed by a parent. Pages 3-4 are to be completed by your child's physician. The Asthma Treatment Plan is also to be completed by the physician IF your student has asthma. Sports physical forms are located in the main office on the counter top, or you may print them from my website (above). At the middle school level (and high school level) it is more convenient to have any school physical exam completed on the Athletic Pre-Participation Physical form (Packet #1).
    Sports physical reports are good for 365 days from the date of the exam.
